El núcleo subtalámico y su potencial utilidad como blanco de estimulación cerebral profunda en trastorno obsesivo-compulsivo

El trastorno obsesivo compulsivo (TOC) representa una patología neuropsiquiátrica en la cual, la existencia de un grupo de pacientes con síntomas severos y refractarios a la actual terapia farmacológica y psicológica motiva la búsqueda de nuevas alternativas terapéuticas. En este contexto, la neuroestimulación quirúrgica del núcleo subtalámico (NST), una estructura de fundamental importancia en el control de los circuitos fronto-estriatales de procesamiento motor y límbico, vinculada a la fisiopatología del TOC, ha sido postulada como un tratamiento prometedor para dichos pacientes. Es por ello que el objetivo de la presente revisión se centra en presentar la evidencia clínica actual relacionada a la efectividad de la neuroestimulación en NST para control de síntomas de TOC, así como plantear las controversias existentes en cuanto a posibles eventos adversos psiquiátricos que deberán ser cuidadosamente evaluados en los futuros estudios que se realicen al respecto.
