Severe chronic occlusive arterial disease and other vascular diseases, better yet, cases associated with critical ischemia unresponsive to pharmacological and surgical therapies are an important indication for spinal cord stimulation (SCS). This therapy represents a unique opportunity to influence the physiology and neuronal activity, therefore generating increased distal blood flow to significantly relieve pain and improve the quality of life in the long term. The SCS is a relatively straightforward procedure in experienced hands. After selecting the patient, the stimulating electrode is placed in the epidural space under local anesthesia and fluoroscopic guidance and produces paresthesias in the metameric territory transmitter of pain impulses. It is a safe technique for the treatment of patients with critical limb ischemia due to vascular occlusive disease, since it reduces its morbidity and mortality with low rates of complications.
