Abstract

Introduction: The stereotactic-guided craniotomy and microsurgery is performed with the aid of a computer-driven system where neuroradiological images are acquired and then represented in three dimensions to optimize the resection of lesions in complex locations.
Objective: To describe the experience with use of stereotactic-guided surgery to perform craniotomy in 33 patients with brain lesion.
Materials and methods: Retrospective and descriptive study, reviewing medical records, protocols, and biopsies of patients between 2005 and 2010.
Results: All patients in this study 54.5% were male. The average age of the sample was 49.8 years. Most lesions were located in the frontal lobe with 49%. Headache was the most common symptom with 46.8% of the patients. According to histological type the majority corresponded to tumors, where meningiomas reach 21%. There was no mortality in this study.
Discussion: The reappearance of stereotactic technologies with frame has stimulated the development of guided surgery by images, acquiring great importance in the managing of cerebral lesions of difficult access. In addition, the precise location of injuries allows to minimize incisions when you perform centered craniotomies diminishing to the minimum the cerebral retraction.
Conclusions: Safe and effective technique with ac- curate resection of lesions under direct vision, small bone opening and transcortical route planned. Minimizing postoperative morbidity, hospital stay, optimizing resources for patients and hospitals.
