A case of a patient with intense hemiparkisonian tremor was described. The patient was submitted to a caudal zona incerta (ZI) radiofrequency microlesion with regular result. In same the operative session, a pallidotomy was added, and the tremor was totally abolished. Here we described briefly the surgical procedure and discuss the role of globus pallidus internus (Gpi) in Parkinsonian tremor.
